How Effective is your Maintenance Error Management System (MEMS)?

The basic MEDA philosophy is based on an understanding that people do not intentionally make errors and that organizational factors play a significant and contributory role.

It is widely recognized that the majority of causal and contributory factors leading to an error can be managed. In addition developing root cause understandings, leads to a wider and more effective mitigation process.

Where do you Stand?

How many of the following questions can you answer?

a) What is the difference between a Hazard & a Risk?
b) What is the difference between Risk & Exposure?
c) What is the difference between Direct Cause - Root Cause & Contributing Causes?
d) Can Direct Cause & Root Cause be the same?
e) How informed are you regarding the effectiveness of your internal reporting system?
f) What is your level of reporting?

i/ Low - I typically report 2 or 3 times each year
ii/ Med - I typically report 3 to 5 times each year
iii/ Hi - I typically report more than 5 times each year

g) How could the level of reporting within the organisation be improved?
h) Is Boeing MEDA considered a proactive or reactive system now?
i) Same question as above (h) but this time when it was introduced in the mid 90’s - Proactive or Reactive?
j) So why is there a difference?
